Q As per your mail pre-procedure evaluation will be done by Dr. Anand Shroff. Who will carry out the Surgery?
A

Dr Anand Shroff not only performs the procedures himself, but also the pre LASIK checks where we collect your data for the wavefront treatments.

Q Majority of the patients opt for Lasik or EpiLasik (No-cut) ?. Unless majority of the patients opt for Epilasik, how do we ensure its post surgery results? Will Epilasik gives better eye sight than Lasik?
A

This is ususally not an option, that patients decide, unless they were to join Army / Navy etc where LASIK is disallowed. Then EpiLASIK is opted for voluntarily. But the usual course is when Dr Shroff examines you and the data collected  during the check up indicates if LASIK is unsuitable. Some reasons like dry eyes, thin corneas etc, where LASIK is unsuitable, can still be treated with EpiLASIK.  So, this usually comes out of the check up with Dr Anand Shroff. The vision post procedure remains the same, whether LASIK or EpiLASIK. The only difference is in an initial disposable kit, even the laser applied (Concerto 500 hz)  is the same. Q We would like to know if can we conceive within 3 months after this surgery? or will it have any adverse effect?
A

Yes, this should not be an issue. LASIK has no long term effects that affect pregnancy. It is sometimes that pregnancy hormones can affect immediate healing, so we usually advise to try conception after this immediate period of about 6-8 weeks is over.

Q How many days we need to be in Mumbai for post surgery check ups.
A

Usually, if planned well, LASIK done on Saturday, you may leave India on Monday/ Tuesday.

Q Which technique will be better suited for patients with dry eyes? I suffer from dry eyes and have to use lubricating eye drops and gel on a daily basis. The degree of dryness has not been investigated here in London. What is the best procedure in this instant and is she more prone to complications or infections post surgery due to dry eyes?
A

Dry eyes need to be examined before LASIK. If one does LASIK on a pre existing condirion like dry eyes, yes, the dryness can get aggravated. However, many centres do not do a detailed analysis of this prior to LASIK.
If we find you having dry eye from our quantitative tests and qualitative Tests like Schirmers and TBUT (Tear Break Up Time), then you are graded and marked as mild, moderate or severe dry eyes with a scoring and a certain TBUT time is specficed. Then the treatment is planned accordingly. If moderate or severe, LASIK can definitely not be done first. This needs to be first treated and then monitored to see if the score is improving after the treatment.

If there is mild dry eye, or you have an occupational situation wherein we find that you are in a high probability of getting dry eye in future, then EpiLASIK (a modification) in intial steps of LASIK is then advised.

Dr Shroff will be conducting this examination himself and will be in the position to tell whats is best only after this initial check up.

To give the difference between EpiLASIK and LASIK...

In LASIK, the reason for aggravating dryness is because there is necessity to create a 'flap' on the cornea. This disturbs the corneal nerve supply and may cause dry eyes post treatment. Usually being temporary.

However, in cases where we would like no aggravation of already existing dry eye situation which is in control, then we prefer doing LASIK with no flap being made (No cut, no flap LASIK- Bladeless),.

The only downside being a longer time period in recovery, about 4 days more than LASIK and need to use specific eye drops for 3 months.

Q I have had slight increase in my prescription while my sister has had a reduction in her prescription. Does this affect the likelihood of successful 20/20 vision? Do the cylindrical numbers affect the success of the surgery? For how long do the numbers have to be stable before the surgery?
A

he change in prescription is in the range which is acceptable. The change in powers within 0.75D is fine, so long as we do not find any abnormality (this again is checked up by Dr Shroff).

Also, the usual cause of this increase is due to long hours on the PC. After LASIK you will have to take specific instructions from Dr Shroff and even wear a computer prescription glass to avoid getting any shift in powers later.

Cylindrical errors upto -6.00D are corrected by our laser-The Concerto. Usually we accept a change of 0.75D, but any other change should be checked up and powers stabe for a year before considering LASIK.

Q Are there any precautions that we need to take before attending the clinic such as not wearing contact lenses or using nay particular eye drops etc?
A

Not on eye drops etc, but no contact lenses (soft) for at least 4 days prior to check up.

Q We have both taken isotretinoin to treat acne. Tasneem took the medication last year and I completed the treatment this February. We are other wise fit and health and have no other health problems other than dry eyes. Will this impact the surgery at all.
A

Sometimes, taking hormonal medication for acne for long term (not Isoretinoin) can cause a change in cornea, causing shift in powers.

Q My number has increased by 0.25 in the last year. Would this have any bearing on his intention to do Lasik?
A

No, this will not. Do not be surprised if your numbers too may be found different when we examine ourselves as well! Upto 0.75 D is acceptable as a change due to human factor as no person checks their powers with the same centre, same doctor etc. In fact on the same day if you were go to two different opticians, quite possible that you are prescribed slightly different power.

What's important for us is that Dr Anand Shroff checks the powers himself, three times - with dilation, with the aberrometer and then the data that is taken is not manual, but from the wavefront analyzer. So, do not worry regarding this.

Q Is your center affliated to any of the laser surgery center in USA?
A

Ours is one of the only eye hospitals to have got the Joint Commission Accreditation from USA, the JCAHO international body which certifies hospitals outside USA to be on par with US standards of care.

However, there is no affiliation to any other centre outside Mumbai..

Q Do you provide a gaurantee the laser surgery to be risk free?
A

I understand your concern regarding this treatment, but in our experience, using our Concerto Laser and Dr Anand Shroff's expertise, we have never faced any issues which you read about like permanent night vision issues etc.

Offering 'guarantee' is an example of how some centres try to ‘rope in’ patients and therefore a good reason to go to a reputable eye surgeon for LASIK.

Q Why is the Concerto not in the US?
A

This is a concept laser and only two were made initially to show the world the future of laser technology. The company finally decided on making 17 of which Japan has nearly half, which makes it 10 centres worldwide. They are sold out so there is no question of selling any more, whether in the US or elsewhere.

Q Does the  US FDA have to approve it first? Does it take a long time for the USFDA to approve a medical device like the Concerto?
A

Due to domestic market protection, the US FDA has been slow to approve of German lasers. For example, the earlier laser we used- The Wavelight Eye Q, was used by us for 4 years before being launched in the US. Now this is a popular laser in the USA.

Q Why is there only 6 in the world? I have found one the Netherlands, one by a Japenese surgeon in Tokyo, one in Yorkshire Northern England, and I do not know where the others are

Q What does the Concerto Mean by treating and ablating on the same platform? Does that mean its clearly more accurate than the eye q?  My local surgon has an eye q.  Will both lasers give me the same outcome? I would rater have the concerto than the eye Q though.  I know that its 100hz faster than the eye q.
A

We used the Eye-Q earlier from 2004 to 2007. The Concerto definitely has a better eye-tracker and much faster with treatments so lower chance of irregular drying out of the stromal bed and changes in outcome.
